Due to updated mask requirements by the Centers for Disease Control (“CDC”), Alchemy Massage & Bodywork, SA has instituted the following mask requirements. The threat of infection remains. COVID-19 and its variants are contagious. Break through infections are rising. Data shows that “vaccines help keep people with no symptoms from spreading Covid-19” and may help lessen the severity of illness once Covid is contracted. It is possible even with the vaccine to contract Covid-19 and its variants. Please read the following sign, date and bring the signed form with you to your next appointment. You’ll only have to do this once unless there is a change in recommendations, policies, mandates, rules, etc. by the CDC, local, state/federal government or bodywork associations, or my insurance provider.

1. A Mask is required during your appointment. Whether you have been fully vaccinated or not, wearing a mask has been deemed necessary by bodywork associations and my insurance provider. Your face must remain in the face cradle at all times while face down. While face down you may remove the ear loops from behind your ears for comfort. When face up, it will be necessary for you to wear your mask.

2. Temperature checks are discontinued, unless you are showing symptoms of fever or illness.

3. Clients are required to wash their hands or use hand sanitizer prior to their appointment.

4. All cleaning, disinfection and sanitizing, air purification, hours of operation and number of clients seen in a day remain the same.

Clients traveling within or outside of Texas prior to their scheduled appointment upon returning home must self-isolate for a minimum of 8 days before their appointment.  If you are unable to meet the self-isolation requirements, please contact me to reschedule your appointment.  If during self-isolation you have signs of Covid-19 or test positive for the virus, you must cancel your appointment. Prior to scheduling your next appointment please contact me directly.

These conditions are to protect you, other clients and me and your honesty is imperative. If you are a new client or have not seen me within the last 6 months, please be sure to complete and bring signed Covid -19 Mask Requirement Form (June 27, 2021) with you to your next appointment.
